• Fairfield Area School District Medication Procedures and Guidelines
    Prescription and Non-prescription Medication
In accordance with recommendations of the Pennsylvania Department of Health and this school’s medication procedures, students will be given prescription medication and non-prescription medications (other than Tylenol, Ibuprofen, Antacid, and Benadryl) only on the direct written order of a physician and a completed Medication Permission Form, which can be accessed HERE.

    Prescription and Non-Prescription Medications

    • All medications must be brought to the school nurse at the beginning of the day in the original pharmacy bottle/non-prescription package.

    • A parent/guardian or a responsible adult designated by the parent/guardian should deliver all medications to the school. The parent assumes all responsibility for medications sent to school. When someone other than the parent/guardian delivers medication to school:

        • The container is to be placed in a sealed envelope.

        • Medication is to be delivered to the health office and/or school office if the nurse is unavailable at the time of delivery.

    • Student’s first and last names must be written on the original, non-prescription package.

      • Medications not in their original pharmacy bottle/non-prescription package will not be used.

      • A completed Medication Permission Form must accompany medication before it can be given by the school nursing staff.

    •

    Asthma Inhalers**
Students are permitted to carry inhalers during the school day and after-school activities once evaluated by the school nursing staff for competency in self-administration of the inhaler.  A Medication Permission Form must be completed and returned to the school nurse prior to use of inhaler at school or during after school activity (school sponsored).  Additional requirements for asthma inhalers:

    • Student must report the following to school nursing staff:
      • Use of inhaler during the school day
      • If symptoms do not improve with inhaler use
      • Any side effects
    • Students are responsible for maintaining the medication at school.
    • Inhalers must be labeled with the student’s name (first and last).

**After school activity supervisor must be aware of the location of medication during the activity.
**Student must provide a back-up medication to be stored in the nurse’s office.

    Please Note:

      • The student is responsible for coming to the health room to take their medication.

      • The school nurse will work with parents/guardians to ensure medications are picked up by the parent/guardian at the conclusion of the school year.

      • If the medication is discontinued prior to the last day of school, the remaining medication will be picked up by the parent/guardian.  Epi-Pens and Inhalers cannot be picked up before the last day of school.

      • Parents may not request a controlled substance to be taken to and from school. If a parent requests for medication to be taken to and from school, the parent must submit this in writing to the principal and school nurse. These requests will be considered and approved by the school nurse and building principal on a case-by-case basis.
